Appointment Guide
It is important to record your appointments, write down information regarding your progress and treatment, and list any new questions you may have for your health care team. Print and take this Guide with you to doctors visits, treatment sessions or other important appointments.

In addition to your cancer therapy, you may be taking other medications, vitamins or supplements.
